📋 Description
- Provide home health visits to adult/geriatric clients in designated NJ territories.
- Perform nutrition assessments and create personalized care plans.
- Deliver medical nutrition therapy for disease management and prevention.
- Counsel clients/families on nutrition per health goals.
- Educate clients and clinical team on nutrition strategies for home use.
- Document care and progress in BAYADA EMR accurately.
🎯 Requirements
- Bachelor's degree or higher in nutrition/dietetics.
- Current RD/RDN credential; NJ practice eligibility.
- Minimum 2 years RD experience in healthcare setting.
- Home health experience preferred; not required.
- Strong communication and time management skills.
- Experience with EMR is a plus.
