📋 Description
- Provide clinical assistance as part of care teams traveling to senior living communities
- Perform pre-visit planning, chart prep, and patient screenings
- Reconcile medications and maintain accurate patient charts
- Assist with minor procedures under provider supervision
- Manage portal messages and coordinate with providers and staff
- Deliver care that is preventative, proactive, and tailored to patients' needs
🎯 Requirements
- Active LPN license or CMA/RMA/CCMA certification
- Valid driver’s license required
- Experience with electronic medical records and medication reconciliation
- Strong interpersonal and communication skills
- Ability to work in field settings and manage clinical flow
- English language proficiency
