Occupational Therapist

📋 Description

• Provide occupational therapy services to adults and geriatrics in senior living communities.
• Perform comprehensive OT evaluations and develop individualized treatment plans.
• Deliver services in senior living or group home settings with a focus on independence and safety.
• Collaborate with home health clinicians to ensure smooth transitions of care.
• Educate clients, families, and caregivers on treatment plans and home programs.
• Complete timely documentation and discharge planning using EMR.

🎯 Requirements

• Current New Jersey OT license.
• Minimum of one year OT experience.
• Home health experience preferred.
• Graduate of an accredited OT program recognized by AOTA/ AMA.
• Excellent communication, organization, and time management skills.
• Ability to work independently and with an interdisciplinary team.
