Care Coordinator, LSW (MSHO)

📋 Description

• Manage member caseloads within your geographic area
• Coordinate face-to-face visits
• Manage Elder Waiver processes
• Conduct annual assessments for PCA and living tools in assisted living
• Review health needs, set goals, and develop care plans
• Connect members with community/state resources and services

🎯 Requirements

• Current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW) – new grads welcome
• Or Minnesota Registered Nurse license
• 1+ year experience with geriatric or disabled populations in care settings preferred
• Valid driver’s license
• Ability to work independently; strong verbal communication
• English language proficiency
