📋 Description

• Manage and strengthen relationships with referral partners to support patient transitions to
• Focus on account management, education, care coordination, and collaboration with clinical teams.
• Serve as primary contact for referral sources; ensure timely, responsive communication.
• Collaborate with discharge planners, case managers, and other healthcare partners to optimize
• Represent Cabrillo Hospice at community and professional events to support partnerships.

🎯 Requirements

• Experience in hospice, home health, or healthcare account management strongly preferred.
• Solid understanding of hospice philosophy, eligibility criteria, and interdisciplinary care.
• Minimum two years of experience with physicians, hospitals, insurers, SNFs, or other healthcare
• Strong communication, presentation, and relationship-building skills.
• Excellent organizational skills with the ability to manage multiple accounts and priorities.
• Ability to work independently and collaboratively within a multidisciplinary team.
