Weekend Physical Therapist (PT)- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, gait, balance, and safety as part of care
• Implement wound care and prevention using evidence-based dressings and interventions.
• Develop and implement individualized care plans with manual therapy and gait/balance training.
• Assess and revise the PT care plan; collaborate with nursing on the multidisciplinary plan.
• Educate clients and family on rehabilitative care to promote health and independence.
