Home Health Visits Registered Nurse

📋 Description

• Provide skilled nursing care in patients' homes as part of a multi-disciplinary team
• Perform head-to-toe assessments and identify subtle changes in condition
• Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and independently handle urgent clinical situations in the home
• Reconcile medications, educate patients, and monitor safety
• Document wound care and other care activities thoroughly

🎯 Requirements

• Massachusetts nursing license in good standing
• Graduate of accredited nursing program
• At least 1 year of adult/geriatric clinical experience within past 2 years
