Admissions Registered Nurse, RN, Home Health Visits

📋 Description

• Provide 1:1 skilled nursing in patients' homes
• From hospital discharge to independent, safe home life
• Conduct assessments and monitor patient condition
• Educate patients and families on care plans
• Deliver wound care and medication administration
• Coordinate care to maintain comfort, independence, and dignity

🎯 Requirements

• Active PA RN license
• At least 1 year nursing experience
• Prior home care experience required
• Ability to travel to cases as assigned
• Strong clinical and communication skills
• Compassionate and reliable patient care
