Job Description
📋 Description RN in home care; perform clinical evaluations in members’ homes. Conduct monthly visits to ensure quality service and address condition changes. Develop and adjust service plans with the Adult Foster Care program. Coordinate care with physicians, RNs, and other disciplines; document visits. Provide caregiver and member support; follow agency policies and procedures. Travel within the state as needed; maintain patient privacy and compliance. 🎯 Requirements Registered Nurse licensed by Georgia Board of Nursing. Minimum 2 years clinical experience; 1 year with elders or adults with disabilities. Bilingual in Spanish is a plus; CRM experience (Salesforce a plus). Valid driver’s license with an insured vehicle for travel within the state. Strong communication, organization, and problem-solving skills.
