📋 Description Provide community-based care coordination and recovery support for individuals with mental health Serve as primary support and case manager for clients with mental health conditions and SUD. Coordinate services with healthcare providers and community resources to support independence. Assist clients in developing recovery goals and person-centered care plans. Document case notes and maintain service records per program standards. Collaborate with multidisciplinary teams to deliver comprehensive behavioral health care. 🎯 Requirements High school diploma or GED required. Active Washington State SUDP credential required. Minimum six months experience in addictions treatment or substance use setting. Strong interpersonal skills; ability to build trust with clients. Ability to work with a multidisciplinary care team. Commitment to culturally responsive, person-centered services. 🎁 Benefits Competitive pay: approx. $26.46 to $32.69/hour, up to $38.93 with experience/location.
