📋 Description Per diem weekend home health PT for senior living office in Raleigh, NC. Make home visits to clients in Wake & Durham counties. Provide rehabilitative care and skilled nursing to allow clients to stay at home. 🎯 Requirements North Carolina Physical Therapist license. Minimum one year of PT experience. Experience with home health/OASIS documentation required. Independent work ability; strong interpersonal, organizational, and problem-solving skills. Solid computer skills; EMR experience preferred.
