Registered Nurse - RN or Licensed Practical Nurse - LPN Hospice Visits

📋 Description Home visits for hospice patients in designated territories (North Jersey). Provide care per visit; meds, wound care, treatments, comfort care. Monitor patient conditions and report changes to supervisor. Follow Doctors’ orders and document appropriately. Travel to cases as assigned; occasional after-hours/weekends. Per diem, weekday availability; 1-2 holidays per year. 🎯 Requirements Active NJ RN or LPN license. Minimum 1 year clinical (medical/surgical) experience. Home care/hospice experience preferred but not required. Strong computer skills; EMR experience preferred. Ability to travel to cases as assigned.
