Medical Social Worker, Home Health

📋 Description Make home visits to clients in designated geographic territories. Assess social and emotional factors related to clients’ health problems. Provide clinical social evaluations and plan interventions. Identify community resource referrals for patient and family needs. Advocate for client and family to navigate the community system. Collaborate with multidisciplinary BAYADA clinicians on the care plan. 🎯 Requirements Must be licensed by the VA Board of Social Work Examiners at LMSW or LISW level. Master's in Social Work with at least 2 years of community-based experience. Accreditation by CSWE (transcript or degree acceptable). Home care experience a plus. Strong reading/writing/communication in English. Ability to work independently and manage time effectively. 🎁 Benefits Base Pay: $52.00-$62.00 per point (information may vary by qualifications).
