Speech Language Pathologist, (SLP) - Home Health

📋 Description Make home visits to clients in designated geographic territories. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients Develop a treatment plan based on evaluation, physician orders, and goals. Continuously assess and revise the care plan with BAYADA clinicians in the multidisciplinary team. Educate patients and families in rehabilitative care to promote independence. Document observations and evaluations in the EMR on a touch pad tablet. 🎯 Requirements Current North Carolina Speech Language Pathologist license. Certificate of Clinical Competence in Speech from ASHA, or equivalent qualifications and Experience or coursework meeting licensure requirements; undergoing ASHA exam is acceptable. Complete related education and supervised work experience for certification.
