Care Manager (Gordon & Whitfield, GA / Field-based)

📋 Description Perform orientations for new patients in the FreedomCare program. Assess patients to determine care needs and training requirements for caregivers. Train caregivers to ensure high-quality, compassionate home care. Coordinate field-based care across Gordon & Whitfield, GA; travel within GA as needed. Engage with interdisciplinary teams, caregivers, and patients to ensure effective care.
