📋 Description Field-based care manager responsible for orientations, patient assessments, and caregiver training Perform clinical and non-clinical tasks to support patient independence at home. Collaborate with caregivers, patients, and care teams to ensure quality home care. Travel within the state to meet patients and oversee program implementation. 🎯 Requirements 2+ years of experience with frail or elderly adults and caregivers (preferred). Excellent communication, organization, and time management skills. Hands-on experience in home care; willingness to travel within Georgia.
