Registered Nurse (RN) - HCV, Full Time or Part Time

📋 Description Provide skilled nursing and rehabilitative care to clients at home as part of a multi-disciplinary Conduct comprehensive head-to-toe assessments and identify changes in patient condition. Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care). Triaging and independently managing urgent clinical situations in the home. Perform medication reconciliation, patient teaching, and safety monitoring. Document wound assessments and care using point-of-care technology. 🎯 Requirements Current nursing license in good standing in the state of Massachusetts. Graduation from an accredited nursing program.
