📋 Description Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status Implement integumentary protective interventions to prevent wound formation and promote repair with Develop and implement individualized care plans, including manual therapies, gait training Continually assess and revise the PT care plan, and participate with nursing in the Educate clients, family members, or representatives in rehabilitative care and activities to 🎯 Requirements Current New Jersey Physical Therapist license. Graduation from an APTA/CAAHEA-approved program, evidenced by transcripts/diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills.
