Medical Social Worker, Home Health

📋 Description Make home visits to clients in designated geographic territories. Assess social and emotional factors related to health problems. Provide clinical social evaluations and plan interventions. Identify community resource referrals for patient and family needs. Advocate for clients to navigate the community system. Collaborate with BAYADA clinicians on total care plans. 🎯 Requirements Must be licensed by STATE Board of Social Work Examiners at LMSW or LISW level. Master’s in Social Work with at least 2 years of community-based experience. CSWE accreditation demonstrated by transcript/degree. Home care experience a plus.
