Registered Nurse (RN) - Telehealth Medicare Annual Wellness Visit Specialist - PRN

📋 Description Conduct Medicare AWVs via telehealth per CMS guidelines. Review and update patient histories remotely. Assess health status, vitals, and cognition using remote tools. Educate patients on preventive care and community resources virtually. Develop personalized prevention plans with physician guidance. Document findings and follow-ups in the EHR. 🎯 Requirements Must be bilingual in Korean and English. Must be located in GA. Current and unrestricted RN license in GA. PRN availability Mon-Fri 8:00am-5:00pm (~20 hrs/wk). Minimum 3 years of patient-care nursing experience.
