📋 Description Work remotely with medical practice staff to manage target patient population. Work independently and as part of the care team to support patient care. Help patients achieve better outcomes; connect to care, referrals, and resources. Document patient history and issues in the EHR accurately. Understand referrals and prior authorizations; navigate care pathways. Build trusted relationships with medical practices and Guidehealth while meeting KPIs. 🎯 Requirements Certified/Registered Medical Assistant or EMT with 2+ years experience. Passion for coordinating care for complex medical/social needs. Willingness to learn motivational interviewing and behavioral change. Strong interpersonal skills with empathy and professionalism. Excellent written/verbal communication; able to document interviews in real time.
