📋 Description Lead HEDIS adherence and related quality measures. Analyze adherence data; identify gaps; implement interventions. Collaborate with providers and care managers to promote adherence. Conduct MTM sessions; educate patients; address barriers. Monitor HEDIS metrics; generate progress reports from EHR data. Serve as SME; train pharmacists and staff; join cross-functional teams. 🎯 Requirements PharmD degree; active pharmacist license in TX, PA, AL, or TN. Minimum 2 years in HEDIS-related adherence, QoI, or population health. Strong knowledge of HEDIS measures related to medication adherence. Proficient with EHR systems and data analysis tools. Excellent communication, problem-solving, and organizational skills. Motivational interviewing experience a plus. 🎁 Benefits Fully remote; work from home.
