📋 Description Lead HEDIS medication adherence initiatives to improve measures. Analyze adherence data, identify gaps, and implement interventions. Collaborate with providers, care managers, and stakeholders to promote adherence. Conduct MTM sessions, educate patients, and address barriers. Use EHR and pharmacy data to identify at-risk patients and plan actions. Monitor HEDIS performance and report progress to leadership. 🎯 Requirements PharmD degree from accredited school. Active pharmacist license in TX, PA, AL, or TN. 2+ years in HEDIS adherence work or quality improvement. Strong knowledge of HEDIS measures for adherence.
