📋 Description Work remotely with care teams to manage patient populations. Collaborate with medical practices as an extension of care. Assist patients, address questions, and arrange referrals. Document patient history accurately in the EHR. Understand referral and prior authorization processes. Escalate medical issues to RN or physicians when needed. 🎯 Requirements Certified Medical Assistant or EMT with 2+ years of experience. Love coordinating care for patients with complex needs. Willing to learn motivational interviewing and behavioral change. Strong interpersonal skills: empathy, professionalism, respect. Proficient with Microsoft Office (Outlook, Word, Excel).
