Registered Nurse (RN) - Telehealth Medicare Annual Wellness Visit Specialist - PRN

📋 Description Conduct Medicare AWVs via telehealth per CMS guidelines. Review and update patient histories and meds remotely. Assess health status, vitals, cognition, and risk factors remotely. Educate patients on preventive care and community resources virtually. Develop and update personalized prevention plans with physician guidance. Document and follow up on identified concerns in the EHR. 🎯 Requirements Must be bilingual in Spanish and English. Must be located in GA with a current, unrestricted GA RN license. PRN, Mon-Fri 8am-5pm; ~20 hours/week. Minimum 3 years in patient care; primary/preventive/telehealth preferred. Experience with Medicare AWVs and telehealth platforms desirable.
