📋 Description Manage member caseloads within assigned geographic area; handle DHS/CMS activities for Minnesota Senior Health Options (MSHO/MSC+) and SNBC; travel to community settings; work from home. Coordinate face-to-face visits; manage Elderly Waiver; conduct annual PCA assessments; customize living tools for members in Assisted Living. Review health needs, identify goals, and develop individualized care plans. Help connect members with community/state resources; complete required documentation. Collaborate with medical care teams to meet quality measures and use utilization management tools; support transitions of care with safe discharge and follow-up plan. Schedule: Full-time, day shift; no evenings, weekends or holidays (8am-5pm Mon-Thu; 8am-3pm Fri). 🎯 Requirements Current MN Licensed Social Worker (new grads encouraged to apply) Or Minnesota Registered Nurse license One or more years of experience with geriatric or certified disabled populations in case management/home care/group homes/nursing homes/TCU/assisted living settings Must have a valid driver’s license 🎁 Benefits Health Insurance Dental Insurance Vision Insurance Company paid Life Insurance Company paid Short and Long-term Disability Health Savings Account (with employer contribution)
