📋 Description Field-based care manager delivering 1:1 patient care in senior living communities. Primary link between patient, family, and PCP to execute unified care plan. Develop and manage individualized care plans; support advance care planning. Implement CoCM behavioral health interventions and collaborate with care team. Close care gaps, optimize transitions, and reduce hospital utilization. Full-time role with day shifts; 90% field-based across Southern Sarasota. 🎯 Requirements Bachelor’s degree or higher preferred; LPN/RN/Social Worker license preferred. 3-5 years in value-based care, population health, case management. Behavioral health experience preferred. Knowledge of Assisted Living and Memory Care communities. Strong time-management and organizational skills; able to work independently.
