📋 Description 1:1 patient management in senior living communities. Primary liaison between patient, family, and PCP. Focus on closing care gaps and reducing readmissions. 90% field-based care across Minnetonka and SW Metro. 🎯 Requirements Bachelor's degree or higher; LPN/RN or Social Worker preferred. 3-5 years in value-based care, population health, or care coordination. Behavioral health experience preferred. Knowledge of Assisted Living and Memory Care communities. Independent work with strong time management and organization.
