📋 Description Manage member caseloads within assigned geographic area. Coordinate face-to-face visits and Elder Waiver participation. Conduct annual PCA assessments for community members. Review health needs, set goals, and develop care plans. Connect members with community and state resources; complete documentation. Support transitions of care with discharge and follow-up plans. 🎯 Requirements Current MN Licensed Social Worker (new grads encouraged to apply). OR Minnesota Registered Nurse license. One or more years of experience with geriatric or disabled populations in care coordination preferred.
