📋 Description Field-based care manager providing 1:1 patient management in senior living communities. Primary liaison between patient, family, and PCP; ensures a unified care plan. Optimize outcomes, close gaps in care, reduce hospitalizations. Travel to Assisted Living/Memory Care/Group Homes across Minnesota, Wisconsin and Florida. Collaborate with MDs, NPs, and PAs; update care strategies as patient status evolves.
