Home Health Medical Social Worker

📋 Description Make home visits to clients in designated geographic territories. Provide clinical social evaluations and plan interventions. Identify community resource referrals to address client and family needs. Advocate for client and family to navigate the community system. Participate in the total plan of care with the multidisciplinary team.
