Specialty Pharmacy Liaison

📋 Description Provide comprehensive medication management to hospital patients. Work with clinic team to serve patients face-to-face and by phone. Educate patients on hospital services and care outcomes. Manage outbound therapy and medication adherence, including refills. Communicate with the Patient Service Center about refill reports and appointments. 🎯 Requirements State-registered pharmacy technician; CPhT preferred but not required. 2 years required; 3-5 years preferred. High School Diploma or GED required. Strong interpersonal and written/verbal communication; proficient in MS Office. Knowledge of transplant, hepatitis C, infectious disease, or oncology a plus. 🎁 Benefits Hybrid schedule: 1 day remote per week.
