📋 Description Make home visits to clients in designated territories. Perform diagnostic tests: mobility, ROM, gait, strength, balance. Implement wound care and protective interventions. Develop and implement individualized care plans with manual therapy, gait training, balance. Continually assess and revise the PT care plan with nursing. Educate clients and families in rehabilitative care for health and safety. Accurately document observations and interventions using a touch pad tablet/EMR. 🎯 Requirements A current NJ Physical Therapist - PT license. A minimum of one year of recent work experience as a Physical Therapist - PT. Graduate from an APTA/CAPTE/AMA-approved PT program. Ability to work independently and manage time effectively.
