📋 Description Make home visits to clients in designated geographic territories. Perform tests and measurements: mobility, ROM, stability, gait, strength, balance. Implement wound care and protective interventions with evidence-based dressings. Develop care plans including manual therapy, gait training, balance, and other interventions. Assess and revise PT care plan; coordinate with nursing. Document observations and interventions using EMR. 🎯 Requirements Current Pennsylvania Physical Therapist license. Minimum one year of PT experience. Graduate from an APTA-approved program or AMA CAAHEA. Ability to work independently and manage time.
