📋 Description Make home visits to clients in designated territories. Perform mobility/strength/ambulation assessments. Implement wound care and dressings to promote healing. Develop and update individualized care plans, including exercises. Continually assess and revise PT care plan with the care team. Educate clients and families on rehab and safety. 🎯 Requirements A current NJ Physical Therapist - PT license. At least 1 year of PT work experience. Graduate from APTA/AMA-approved program (transcript/diploma). Ability to work independently and manage time. Strong interpersonal, organizational, problem solving skills.
