📋 Description Make home visits to clients in designated territories. Perform mobility, strength, balance, and safety assessments. Implement wound care with evidence-based dressings. Develop individualized care plans with manual therapy and gait training. Continually assess and revise the PT care plan with nursing. Educate clients and families on rehabilitative care. 🎯 Requirements A current New Jersey Physical Therapist (PT) license. Minimum of one year of PT work experience. Graduation from an APTA- or AMA-accredited program. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills.
