📋 Description Make home visits to clients in designated geographic territories. Perform mobility and strength assessments; monitor ambulation and safety. Implement wound care with evidence-based dressings and topical agents. Develop individualized care plans including manual therapy and gait/balance training. Educate clients and families in rehabilitative care and independence. Document observations and care using a touch-pad tablet. 🎯 Requirements North Carolina Physical Therapist license. Minimum 1 year PT experience. Graduate from APTA/CAAHEA-approved program. Ability to work independently and manage time. Strong interpersonal, organizational, and problem-solving skills.
