📋 Description Make home visits to clients in designated geographic territories. Perform evaluations of functional status including ADL/IADL. Assess cognitive, psychological and motor performance; develop intervention plans. Evaluate home environment for hazards or barriers to independence. Identify equipment needs and environmental adaptations for better outcomes. Continually assess and revise the OT care plan with nursing in multidisciplinary team. 🎯 Requirements A current VA Occupational Therapist license. Prior home care experience preferred, but not required. A minimum of one year of recent work experience as an Occupational Therapist. Graduation from an OT program accredited jointly by AMA and AOTA.
