📋 Description Follow a designated care plan per patient needs. Make home visits to clients in designated territories. Administer medications, wound care, treatments, and procedures. Monitor client conditions; report changes to Clinical or Client Services Manager. Follow up with, execute, and document doctors' orders; perform client assessments. Case management and coordination; document observations in EMR on a tablet. 🎯 Requirements Current NJ RN license. Minimum 1 year recent clinical (medical/surgical) experience. Graduation from accredited nursing program. Prior home care experience strongly preferred. Ability to work independently and manage time; strong interpersonal skills.
