Job Description
📋 Description Conduct PT assessments in facility and in-home to define care needs and goals. Actively participate in IDT meetings to develop participant care plans. Deliver and document interventions per participant care plans. Provide active and passive exercise, gait training, ADL therapy, and modalities. Supervise Therapy Assistants and implement Functional Maintenance Programs (FMP). Provide training to staff, participants, and caregivers on therapeutic equipment. 🎯 Requirements Bachelor’s degree in physical therapy; master's/doctoral preferred. Valid California Physical Therapy License or license eligibility. CPR and First Aid certification within 180 days of hire.
