📋 Description Conduct home health nursing assessments to determine equipment needs Actively participate in IDT meetings to develop care plans Deliver and document nursing interventions per care plans Instruct patients, families, and caregivers on disease, self-care, and prevention Track and monitor home health scheduling with the Home Health Manager Collaborate with physicians and IDT for smooth care transitions 🎯 Requirements Graduate of accredited school of nursing Unencumbered RN license required At least 1 year nursing experience with frail/elderly population; home health preferred Reliable transportation CPR and First Aid certification within 180 days of hire 🎁 Benefits Medical insurance coverage (Medical, Dental, Vision) 17 days PTO, 12 holidays, and 6 sick days 401(k) savings with company match Comprehensive compensation package with base pay and bonus Additional benefits
