Clinical Pharmacist

📋 Description Review clinician notes for prior authorization and care plan recommendations. Develop and refine disease- and drug-specific clinical protocols. Monitor patient adherence (PDC, assessments, mobile data) across sites. Evaluate EMR for drug interactions, labs, allergies, and orders. Recommend guideline-based alternative regimens to optimize care. Medication therapy management (MTM): review regimens and interventions. 🎯 Requirements PharmD or equivalent; licensed Pharmacist in Florida. Florida license required; 1+ year experience preferred. Strong knowledge of specialty pharmacology and regulations. Proficient with Microsoft Office; EPIC/QS1/WILLOW a plus. Excellent communication and patient coaching skills. Ability to maintain HIPAA compliance and confidentiality.
