Clinical Manager, Home Care

📋 Description

• Conduct nursing assessments for geriatric clients in Bethlehem, PA and surrounding areas
• Develop, manage, and oversee client care plans while coordinating communication between clients
• Provide clinical leadership and supervision to Home Health Aides and field staff
• Serve as a mentor and resource for caregivers, helping them develop professionally
• Facilitate orientation and onboarding for new Home Health Aides and Certified Nursing Assistants
• Participate in training, coaching, competency development, and in-service education programs for

🎯 Requirements

• Active Registered Nurse (RN) license required
• Minimum of 2 years of experience as a Registered Nurse required
• Home Care, Home Health, Hospice, Private Duty, Case Management, or Community Health experience
• Supervisory, Charge Nurse, Clinical Lead, Preceptor, or Nurse Manager experience preferred
• Strong assessment, care planning, and clinical decision-making skills
• Excellent communication, organizational, and leadership abilities
