Physical Therapist – Weekend SOC Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status
• Implement integumentary protective interventions to prevent wound formation and promote wound
• Develop and implement individualized care plans including manual therapeutic exercises, gait
• Continually assess and revise the physical therapy care plan and participate in multidisciplinary
• Educate and instruct clients, family members, or other representatives in rehabilitative care and

🎯 Requirements

• Current Maryland Physical Therapist license
• Minimum of one year of recent work experience as a Physical Therapist
• Graduation from an APTA or CAAHEP approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; prior EMR experience preferred
