Clinical Manager, Home Care

📋 Description

• Lead, inspire, and motivate staff to provide exceptional client care
• Visit Assistive Care clients and conduct assessments throughout Montgomery and Delaware counties
• Develop individualized care plans and oversee their implementation
• Supervise, coach, and support Home Health Aide (HHA) staff in the field
• Facilitate new hire orientation and onboarding
• Conduct competencies for new and tenured staff

🎯 Requirements

• Active Registered Nurse (RN) license
• Minimum of 2 years of RN experience
• Recent hands-on nursing experience required
• Prior home care experience preferred
• Previous supervisory or leadership experience preferred
• Strong clinical judgment, communication, and organizational skills
