Physical Therapist (PT) - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of joints, transfer status, stability
• Implement integumentary protective interventions for wound prevention and repair
• Develop and implement individualized care plans including manual therapeutic exercises, gait
• Continually assess and revise care plan, participate in multidisciplinary plan
• Educate clients, family members, or representatives on rehabilitative care and activities

🎯 Requirements

• Current North Carolina Physical Therapist - PT license
• Graduation from a program approved by the American Physical Therapy Association or the Committee on
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
