📋 Description

• Make home visits to clients in designated geographic territories.
• Perform comprehensive evaluations of functional status and ADL/IADL.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environment for hazards; propose adaptations.
• Identify equipment needs for improved outcomes.
• Collaborate with nursing in multidisciplinary care plan.

🎯 Requirements

• A current NJ Occupational Therapist license.
• Minimum one year of OT experience; home care preferred but not required.
• Graduation from an accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving.
• Solid computer skills; EMR experience preferred.
