RN Transitional Care Manager (Adult and Pediatric Skilled)

📋 Description

• Serve as liaison between referral sources, hospitals, and clients transitioning to home care.
• Reduce readmissions and improve outcomes through transitional care.
• Coordinate admissions and continuity of care with BAYADA offices.
• Educate partners, clients, and families on BAYADA services.
• Develop and execute referral development plans to grow the territory.
• Represent BAYADA at facilities, events, and community Outreach.

🎯 Requirements

• Current, unrestricted RN license.
• Bachelor’s degree preferred.
• Minimum of five years in nursing, discharge planning, home health, or related areas.
• Experience in critical/acute/long-term care and transitional care preferred.
• Strong relationship-building with healthcare professionals.
• Strong communication and interpersonal skills; ability to travel extensively.

🎁 Benefits

• Base Salary: $95,000 - $110,000 annually.
