- Lead an interdisciplinary hospice team focused on patient‑centered end‑of‑life care.
- Provide clinical guidance and mentorship to promote strong patient/family experiences.
- Cultivate excellence, accountability, teamwork, and professional growth.
- Engage in Performance Improvement, QA, and regulatory readiness activities.
- Develop future hospice leaders through education and coaching.
- Support care planning and problem‑solving for complex clinical situations.
🎯 Requirements
- Passion for hospice and end‑of‑life care; previous hospice experience required.
- Proven leadership in hospice, home health, or palliative care.
- Ability to lead, coach, and develop interdisciplinary teams.
- Knowledge of hospice regulations, compliance, and quality initiatives.
- Experience managing patient caseloads and coordinating care.
- BSN or Master’s degree preferred; RN required.
🎁 Benefits
- Base Salary: $100,000 - $110,000 annually (varies by qualifications).
