Job Description
📋 Description
- Remote role supporting high-risk patients in the 30 days after discharge.
- Provide short-term, telephonic, interdisciplinary care management to prevent readmissions.
- Build individualized care plans addressing medical, psychosocial, medication, and resource needs.
- Collaborate with nurses, physicians, social workers, and community partners to navigate care
- Educate patients and caregivers and advocate for safe, effective transitions of care.
- Operate in a technology-enabled environment focused on outcomes and patient experience.
🎯 Requirements
- Unrestricted U.S. Compact RN license; able to obtain additional state licenses as needed.
- 3–4 years RN care management or telephonic case management experience.
- Available Mon–Fri, 8:30am–5:00pm MST/PST; prefer CT/MT/PT time zones.
- Strong clinical assessment, analytical thinking, and care-planning abilities.
- Clear, compassionate communication of complex medical information.
- Comfort with spending substantial time on the phone building trust and coordinating care.
